Submitting your CMRA forms

As an operator, you must submit your CMRA forms to your local Postmaster.

As an operator, you must submit the CMRA forms on a quarterly basis and on alphabetical listings to the Postmaster of your region. You can find your local Postmaster here.

You must include CMRA forms for:

  • All new customers

  • Current customers

  • Customers who terminated your service within the past 12 months. You must include the date of termination. For terminated members, the PS Form 1583 must be submitted as well and needs to include the termination date.

Quarterly updates are due on January 15, April 15, July 15, and October 15.

CMRAs must retain the endorsed duplicate copies for at least 6 months after the termination date.
